Payment of Employees’ Provident Fund Benefits upon receiving a Permanent and Pensionable appointment in Public Service

Note: It is mandatory to have terminated the employment at the time of claiming benefits.

The completed application form should be personally submitted by the member to the nearest District Labour Office or Sub Labour Office in their residence during the office hours of a working day.

The service will be provided within 3 weeks if all required documents are submitted promptly.

The certified application form endorsed by your most recent employer
“B” cards containing relevant membership numbers
National Identity Card along with a certified copy endorsed by the employer ( In the absence of the National Identity Card, a valid driving license, a valid passport or an Adult identity card can be submitted)

Original letter of appointment to confirm the permanent and pensionable nature of the position and a copy
Service certificate obtained within the last three months from the current institution’s head
A recent Salary report
Employee identity card and a copy (For individuals with a background in the Sri Lanka Armed Forces, a formal service certificate [confirming the permanent and pensionable nature of the position], a recent salary report and employee identity card and a copy are required.)
For the disbursement of benefits under 23/E to members employed in Local Authorities, submission of a letter allocating 60% and 40% (with separate references to time and amount) or a letter recommending 100% payment to the applicant is necessary.
