Payment of Employees’ Provident Fund Benefits upon receiving a Permanent and Pensionable appointment in Public Service

• “தொடர்புடைய உறுப்பினர் எண்கள் கொண்ட "B" அட்டைகள்
• வேலையளிப்பாளரால் அங்கீகரிக்கப்பட்ட தேசிய அடையாள அட்டையுடன் ஒரு சான்றளிக்கப்பட்ட நகல் (தேசிய அடையாள அட்டை இல்லாத பட்சத்தில், செல்லுபடியாகும் ஓட்டுநர் உரிமம், செல்லுபடியாகும் கடவுச்சீட்டு அல்லது வயது வந்தோர் அடையாள அட்டையைச் சமர்ப்பிக்கலாம்)
• வங்கி கணக்குப் புத்தகத்தின் நகல்
• Original letter of appointment to confirm the permanent and pensionable nature of the position and a copy
• Service certificate obtained within the last three months from the current institution’s head
• A recent Salary report
• Employee identity card and a copy (For individuals with a background in the Sri Lanka Armed Forces, a formal service certificate [confirming the permanent and pensionable nature of the position], a recent salary report and employee identity card and a copy are required.)
• For the disbursement of benefits under 23/E to members employed in Local Authorities, submission of a letter allocating 60% and 40% (with separate references to time and amount) or a letter recommending 100% payment to the applicant is necessary.
