Note: It is mandatory to have terminated the employment at the time of claiming benefits.

The completed application form should be personally submitted by the member to the EPF claim branch in the department of labour head office, during the office hours of a working day.

The service will be provided within 3 weeks if all required documents are submitted promptly.

To complete the application process, please provide the following documents:

The certified application form endorsed by your most recent employer
“B” cards containing relevant membership numbers
Copy of the bank passbook
Valid permanent residence visa and a valid passport with copies
If a claim application on behalf of a member holding permanent residency in a foreign country is submitted by another individual, the copy of the permanent visa and passport must be certified by the Sri Lanka embassy in that specific country. Additionally, the fingerprint and signature on the authority-granting letter and on Form ‘K’ should be certified by the Sri Lanka embassy in that respective country.
