For employers

1. Submit two original copies of completed “Form D” as an employer.I. If employees are 10 or fewer, include two certified copies of “Form D” annexure with an official seal.
   II. For organisations having over 10 employees, provide a detailed list including full name, age, nature of employment, wage, and appointment date.
   III. The certifier of “Form D” should also certify and affix the seal.
   IV. Certification of “Form D” or annexures should be done by the owner or a partner; if someone else signs, a power of attorney is required.
2.  Explain reasons in writing, if the request to register under the Employees’ Provident Fund is more than two years from the institution/business commencement.
3. A certification from the company’s board of directors assuring that employees appointed through the transfer of personnel will uphold the continuity of their statutory rights.
4. In the registration process of a company or partnership business, if a director or partner is enumerated in Annexure D (list of employees), the forms must be duly attested by any other director or partner within the organisation, excluding those specifically identified in Annexure D. (This situation poses no complications when the names of all directors or partners are provided in Annexure D)

 

Additional Requirements based on Business Type:

Individual Business (Sole Proprietorship):
• Photocopy of Business Registration certificate
• Photocopy of the owner’s National Identity card

Partnership Business:
• Photocopy of Business Registration certificate
• Photocopies of National Identity cards for all partners

Limited or Private Limited:
• True copy of Business Registration certificate or certificate of incorporation
• Copy of form 01, 05, or form 20, 40 with board information, certified by the company registrar

Foreign Company:
• True copy of Business Registration certificate or certificate of incorporation
• Copy of form 44, 45, and 46, certified by the company registrar
• If the signer of “Form D” is not listed in form 45 or 46, include a copy of the power of attorney
• True copy of the National Identity Card or passport of the person holding power of attorney

Organisations Connected to Government Institutions, Authority, or Project:
• Copy of the document with cabinet approval for the project
• Copy of the letter of staff approval from the Department of Management Services
• Copy of the director and chairman’s project appointment letter
• Copy of the letter of treasury approval
• Submit a written request from the Director and Chairman of the project for registration in the Employees’ Provident Fund
