If employees are 10 or fewer, include two certified copies of &#8220;Form D&#8221; annexure with an official seal.<br \/>\nII. For organisations having over 10 employees, provide a detailed list including full name, age, nature of employment, wage, and appointment date.<br \/>\nIII. The certifier of &#8220;Form D&#8221; should also certify and affix the seal.<br \/>\nIV. Certification of &#8220;Form D&#8221; or annexures should be done by the owner or a partner; if someone else signs, a power of attorney is required.<\/li>\n<li>\u00a0Explain reasons in writing, if the request to register under the Employees&#8217; Provident Fund is more than two years from the institution\/business commencement.<\/li>\n<li>A certification from the company&#8217;s board of directors assuring that employees appointed through the transfer of personnel will uphold the continuity of their statutory rights.<\/li>\n<li>In the registration process of a company or partnership business, if a director or partner is enumerated in Annexure D (list of employees), the forms must be duly attested by any other director or partner within the organisation, excluding those specifically identified in Annexure D.
