Payment of Employees’ Provident Fund Benefits upon receiving a Permanent and Pensionable appointment in Public Service

Eligibility
Should be a permanent and pensionable post.
N:B: It is compulsory to have been left the employment at the time of claiming benefits.
Method of submitting application

Counter on the ground floor of the Head office, all District offices and Labour sub offices, and here

Time to submit application:-
Completed application form should, by the member himself/herself, be handed over to the nearest District labour office or labour sub office in the district where he/she resides, during the office hours of a working day.

No service charge
Period taken to provide the Service (Ordinary Service and Priority Service)
03 weeks, if all documents are completed
Necessary Supporting documents

Application certified by last employer, “B” cards relevant to membership numbers, National Identity Card and a copy of a National Identity Card certified by the employer, copy of the bank pass book; and

Original letter of appointment in support of the appointment is permanent and pensionable

Service certificate obtained within preceding 03 months by the head of the presently serving institution, salary report and employee identity card

Persons who entered to Sri Lanka Armed forces are required to submit formal service certificate (certifying that post is permanent and pensionable) salary report and employee identity card

When release benefits under 23/E to members in Local Authorities, letter of allocation of 60%, 40% (Letter of recommendation of payment whether time and reference 60% 40% separately or 100% to the applicant)

Exceptions, instances not included above and special information
If there are shortcomings in the documents, such matters can be discussed with the Deputy Commissioner of Labour or Assistant Commissioner of Labour of the Labour office to which the application is handed over.
